        DESCRIPTION

    The Express Scripts Mail Order Program is available for members, pensioners and their eligible dependents who use chronic long-term medications. A maximum 90-day supply of your prescription is shipped directly to you for only a $10.00 generic or $30.00 brand co-payment for each prescription. There will be a $150.00 co-payment on all injectible prescription drugs, except insulin.

        WHAT IS COVERED.

    Your mail order service covers all medications which are covered under the regular prescription benefit. The service may be used to purchase any prescription drug but its primary aim is to offer maintenance drugs used for chronic ailments such as high blood pressure, heart conditions, diabetes, asthma, arthritis, etc.

        WHAT IS NOT COVERED.

    Any item not covered by the regular prescription drug benefit coverage.

        HOW TO USE THE SERVICE

            1 . Tell your physician that you have a mail order pharmacy program and ask for a new prescription if it is a medication that you will be taking on a regular basis. Up to a 90-day supply can be dispensed.

            2. Insert the new prescription in the mail order envelope provided.

            3. BE SURE to complete as much of the information requested as possible. YOU MUST at least provide your social security number, name of the person or persons for whom you are sending prescriptions, and the address to whom the medication should be sent. It is to your best benefit to provide any allergy or history information so that the pharmacist will be aware of any potential drug conflict.

            4. You must insert a co-payment for each prescription or you can provide a MasterCard, Visa or Discover card number. If you request an "A" rated generic, you will pay a $10.00 co-payment. If you request a brand drug you will pay a $30.00 co-payment.

    Your medication should be delivered by United Parcel Service or by U. S. mail within two weeks from the date your prescription order was received by Express Scripts.
